Casting Sheets for Principal Actors and Extras

This template casting sheet is used to record the basic information for a principal actor or extra. The form provides for the individual’s contact information, physical description, such as height, weight, eye color, etc., and information on individual’s agent if applicable. There is also a section where a photo of the individual is to be attached. This generic form can be modified to fit the needs of a production company or other business that is seeks to hire principal actors or extras.
