This Model Release is a template agreement whereby a model assigns and grants
permission to a photographer to license and publish all photographs and images of the
model. In this short-form document, the model grants a general release, releasing the
photographer or company from all liabilities and causes of action. This license and
general release agreement can be modified to fit the needs of the drafting parties and
should be used photographers or publishers seeking a general release from a model.
MODEL RELEASE
FOR GOOD AND VALUABLE CONSIDERATION, the undersigned (hereinafter referred to
as the “Model”) hereby assigns and grants permission to _____________________ (hereinafter
referred to as the “Photographer”) to license and publish all photographs and images of the
Model, including the use of the Model’s name and stage name, in the media for any purpose
which may include but is not limited to, advertising, promotion, marketing, packaging for any
product or service or any other lawful purposes.
THE UNDERSIGNED hereby, in good and valuable consideration, the receipt and adequacy of
which is hereby acknowledged, do hereby remise, release and forever discharge the
Photographer and each of its/his/her successors and assigns, from all manner of actions, causes
of action, suits, debts, liens, dues, accounts, bonds, covenants, contracts, claims and demands
whatsoever which heretofore may have been or may hereafter be maintained by the Model,
directly or indirectly, as a result of any matter whatsoever;
THE MODEL further agrees not to make claim, sue or continue suit against any other person or
corporation which may or does claim contribution or indemnity, at law, in equity, or by statute,
against the Photographer, and further undertakes to indemnify the Photographer in respect of the
making or continuation of any such action, claim, demand or proceeding.
AND IT IS HEREBY DECLARED that the terms of this Release are fully understood and that
this Release is given voluntarily.

CONSENT REQUIRED IF MODEL IS UNDER THE AGE OF MAJORITY
THE UNDERSIGNED, hereby acknowledges that he/she is the parent or legal guardian of the
minor named as the Model. I hereby declare that I have legal authority to execute this Release
on the Model’s behalf. I hereby acknowledge that I have read and fully understand the contents
of this Release, and consent to the Photographer using the said photographs and images as set out
herein.
