This Lease Termination Notice is a document used by a landlord to terminate the tenancy
of a tenant for violating the terms and conditions set forth in the lease. This notice states
the reason or violation for which the lease is being terminated. It also includes instructions
for vacating the premises, payment of rent and the return of the security deposit. This
document should be used by landlords located in Delaware when terminating a tenant's
lease pursuant to the lease agreement.

Pursuant to [STATUTE], I terminate this lease and I request you to refund the rental advance
subject to adjustment of arrears of rent. In case of any delay in making the refund of the rental
advance, you shall be liable to pay interest at _________ percent (___ %) [Instruction: Insert

advance.
You are required to vacate and surrender possession of the premises to the undersigned on or
