(“Tenant”) for the property located at ____ [Instruction: Insert address of property being
leased, including suite number, if any.] pursuant to that certain written lease agreement dated
___ [Instruction: Insert lease date.] by and between ____, [Instruction: Insert Landlord’s
name.] as landlord (“Landlord”), and ____ [Instruction: Insert Tenant’s name.] as tenant (the
“Lease”). Pursuant to ___ [Instruction: Insert applicable Lease section or paragraph
describing Landlord’s applicable obligations.] of the Lease, it is Landlord’s obligation to ____
[Instruction: Insert applicable issue to be fixed.].
Pursuant to Tenant’s Lease obligations [Comment: If Tenant is not obligated to provide notice
pursuant to the Lease of such repair (or default, or otherwise) needs, delete this phrase.],
this letter shall serve as notice to Landlord of the need to _____. [Instruction: Insert necessary
repair to be made. If written or oral notice of such request has already been made, also
state here all previous occasions such requests/notice have been made, including all details
of such requests/notices.]
Optional Language: Pursuant to the applicable Lease terms, upon your failure to remedy
the above-referenced issue, Tenant shall take all necessary steps to remedy same, and shall
accordingly abate the following month’s rent in the amount of such repair.

Optional Language: Pursuant to the applicable Lease terms, upon your failure to remedy
the above-referenced issue, Tenant shall deem Landlord to be in default of the Lease, and
Tenant, in its sole discretion, may pursue any and all permitted remedies set forth in the
Lease for such default.

in the event Landlord fails to remedy the issue at hand.]
