This is a document that is used by a producer to obtain releases from talent or models
in connection with a video or film project. By signing this document, the talent or model
gives the producer permission to use their image perpetually during the exhibition and
exploitation of the project. In addition, the talent releases all parties from any claim for
compensation. This document should be used by individuals or entities in connection
with a film project to obtain release from their talent or models.
Release for Talent and Models in Video and Film
I, ______________________ [insert name] ("Talent") residing at __________________ [insert address],
hereby consent to give and forever grant to ______________________ [insert name] ("Producer") located
at __________________ [insert address], its representatives, licensees, marketers, subsidiaries and all
other related parties or publishers of its promotional materials and their successors and assigns, the right in
perpetuity throughout the universe, and in all now known and hereafter existing media, and in any language,
to use my name (including any fictitious names heretofore or hereafter used by me), physical likeness
and/or voice in and in connection with the production, exhibition, exploitation, merchandising, advertising
and promotion of __________________ [insert description of project] (herein "The Project"). This grant
of rights includes the right to use my likeness in any format, including, picture, a photograph, artistic
rendering, or other reproduction by any medium. All rights in the Project shall be under the sole ownership
of Producer.
Talent also hereby releases all parties from any claim by Talent for compensation. I understand the rights
that I am giving up, and I am satisfied with the terms of this release.

Talent Producer
If the Talent is less than eighteen (18) years of age, complete the following the parent or legal
guardian must complete the following part of the Release.
This is to certify that I as parent/guardian with legal responsibility for Talent, do consent and agree
not only to his/her release, but also for myself, and my heirs, assigns and next of kin to release and
indemnify the Producer from any and all liability arising from my/our minor child's involvement in
granting the above Testimonial Consent and Release to the fullest extent permitted by law.

Talent and Model Video and Film Release
Check List
The check list is provided to inform you about the document in question and assist you
in its preparation.
1. This Talent and Model Release form should be used only to obtain consent for the
photographic, video, and film and likeness issues as listed in the Agreement. This
document should be signed prior to any video, film, or photo sessions and certainly
prior to any use of the Talent’s photo or likeness in these media.
2. If the Talent is a minor, under the age of 18 as a rule, be sure the parent or legal
guardian also signs to acknowledge consent.
3. The Manufacturer should keep the original signed Release. A photocopy should be
given to the person giving the consent for his or her records. If a minor is involved,
give one copy to the minor and another separately to the parent or legal guardian.
