This Health Care Proxy is intended to be used by individuals to appoint a proxy (an agent) to be able to speak and act on their behalf regarding health care decisions and life-sustaining procedures. The document provides for the appointment of a Health Care Proxy in case the individual is unable to speak for him/herself either because he/she is terminally ill, injured, or permanently unconscious.
